I had a very similar issue over the weekend, what I found was there is a little bit of play in the screw holes and you have to get it in the exact right spot, not too close or too far away, too close...

To put a glass bed on you then need to print out a shim the same size as the glass to go in the z-axis, for 3mm glass something like this is what you need http://www.thingiverse.com/thing:1474097
