The Dominica Health Questionnaire (DVMQ) is a web-based questionnaire that the Dominican Republic government has designed specifically to manage the possible risks associated with COPD (Chronic obstructive pulmonary disorder). This questionnaire will help authorities to identify you if you or your companions have been exposed to Coronvirus. You will help to raise awareness for your community and others by answering truthfully. If you do not choose to answer the questionnaire, you will be contributing to the Dominican Republic's medical and health care needs.
