0.2 nozzle hell

So my ender 3 with manual leveling isnt printing well at all. Actually the prints are horrible. Somehow it under extrudes on the first layer, yet builds up weird and ends up knocking the prints loose. I tried 2 different brands of filament overture and sugloo? This printer is dialed with cura and a 0.4 nozzle. I tried leveling it tight, loose, perfect. Not much difference. Help.
