What Causes This?

Question. Do nozzles "Wear out"? I do have brand new nozzles to install but wonder why this happens. Bed leveling is spot on

nozzles can wear out - but I have no idea what I'm looking at there.
Maybe get some light on it ?
just can't figure out what it is.

Based on that massive clump, it appears to be extruding a bit heavy.
