Yea, I forgot to mention. When you do that manual extrude make sure the hotend is 10mm or so up off of the bed. I too see that it looks like there is no gap so the filament can't be pushed out.

Manually extruding is nice and smooth, running the program it does this
Increase you nozzle to bed height.
'beerdart' is correct. Make a good gap. If you want, just make the gap way too big and let the filament flop down. If that works, it will be a mess, but then you know for sure it's because your gap was too tight.

I would slice something else. It could be your gCode. Also, it's always a good idea to have at least two slicers at your disposal. That way you can have both do a print and see if they share errors. Download Slic3r or Cura and try to print from them. If the problem is gone then Repetier is likely causing some kind of malfunction.
Also make sure the hob bolt did not eat a groove in the filament preventing further movement. After any jamb or partial jamb i like the remove the filament and nip off a few inches then make sure there are no grindings in the hob groves.
Im running a model that has run fine with no issues till now. The filament is fine in the extruder, its just not extruding correctly. I do have both those slicers though
It is possible you have an internal nozzle clog.
