Stepper Clicking followed by offset layer

Randomly during a print I will hear a clicking sound and than discovery that my X axis has an offset layer. I tried swapping one of the extruder sterpper with the x server and I still have the same issue. What else would cause this?

    Too name a few.

Stepper Motor Current, Belt tension incorrect and slipping, X-Axis gear slipping, X-Axis Binding, debris blocking belt, Bad Bearings.

What should the voltage be at the motor?

I think my tension is good

The setscrew on the belt pulley is tight

Would a bind be a consistent problem or could it happen at random? It seems like the guide rod mounts would have to be broken in order for the guide rods to not be parallel

    Belt and pulley are clean

    Which bearing?

    Thanks

Could be the x-axis stepper motor cord, they all go bad eventually.
